Case Manager - Placement Services

Springfield MCS - Conlon Bldg

Hours:

Full Time – 40 hrs/wk

Summary:

Responsiblitlies:

  1. Ensure compliance (in cooperation with Care Provider) in the following areas:
    *ISP requirements - Prepare assessments for the ISP including goals, strategies, and updates
    *All Health and safety needs of the consumer
    *All Quest requirements and documentation
  2. Routine visits at caregivers’ homes to provide ongoing support and monitoring
  3. Routine visits with the consumer to assess ongoing quality and satisfaction with supports
  4. Coordination of clinical and medical supports in cooperation with the MCS RN, individual, and Care Provider;
  5. Recruitment of direct support staff in cooperation with the individual and Care Provider
  6. Supervision of direct support staff
  7. Ensure the capacity of the program to provide support to Individuals from different cultures
  8. Represent MCS as requested within the community and with other agencies / organizations with similar objectives.
  9. Attends staff meetings regularly and specific training related to individual consumer’s needs.

Required Qualifications/Skills/Training:

The Case Manager must have a bachelor’s degree or a minimum of 5 years of recent professional health-care experience in a related field. Case Manager must also have a valid driver’s license and access to an automobile.
